In advance of our registration dates for MAIN CAMPUS MS & HS STUDENTS on 8/6, 8/7, and 8/9 from 9 am – 3 pm, please click here to read the parent/guardian letter, and complete & bring with you the following forms as needed:
- MS/HS Fees
- MS/HS Code of Conduct
- MS/HS Bus Code of Conduct
- MS/HS Electronic Communications Form
- MS/HS Media Consent Form
- MS/HS Medical Statement
- CSD Federal Program Questionnaire
- MS/HS Sports Exam Form
- HS PII Form (High School only)
- HS Vehicle Registration Form (if HS student is driving to school)

Parents of Returning Students: On registration day please stop by the front table to review, update, and sign your student’s “Student Profile” form. This form shows all of the personal contact, emergency contact, and medical information we have on file. Also bring all forms you completed by clicking the links above.
Parents of students new to Corbett Schools: If you have not done so already, please call the MS/HS office at 503-261-4226 or the District Office at 503-261-4268 prior to the registration dates so we may speak with you.
MAIN CAMPUS MS & HS STUDENTS Registration Dates (parents with students may come any date, any time!):
8/6: Monday, 9 am – 3 pm in the MPB, stage side
8/7: Tuesday, 9 am – 3 pm in the MPB, stage side (sports physicals this day only, from 2-6 pm)
8/9: Thursday, 9 am – 3 pm in the MPB, stage side
ID photos will be taken. Boosters will be on hand to register students for sports. HS students can get lockers. Lunch accounts can be set up.
IMPORTANT: PLEASE BRING SEPARATE CHECKS FOR MS, HS, and MEAL TIME (lunch) PAYMENTS. Boosters sports registration payments will be via debit / credit cards.
